New "Respite Care" Program at Someren Glen Retirement Community

Someren Glen Retirement Community provides the same level of care to their respite guests that they provide to all of their residents.

If you or a family member are looking for respite care due to the need to find a short term solution following a discharge from rehab or the inability to travel with family to out of town events, Someren Glen has an option for you. Often when discharged from a rehabilitation community, people find themselves stuck in a sort of limbo. They are often not quite ready to live independently but no longer qualify for rehabilitation services through Medicare. These folks will often hire some help at home but they only get care a few hours a day and should they have an immediate need when their home health worker is not present, they have no way of getting the assistance they need. Through respite care, Someren Glen provides residents with 24-hour care givers as well as in house therapy services to help continue to strengthen their respite guests. After a week or two, their guests can feel confident moving back home independently without the fear of having another incident and having to return to the hospital and go through everything again.

Respite Care: Includes Base Rate and All Care Services S175 Daily Rate

Basic Services Include:

• A private studio with individual bathroom, convenient kitchenette, generously sized refrigerator

• 24-hour, fully-trained care partners

• Licensed nurses on site daily who can communicate with physicians daily and as needed, ensuring timely and effective communication

• Medication ordering, administration, and tracking by qualified professionals, through community- preferred pharmacy

• Emergency response, advanced safety monitoring and fire systems

• Three delicious, well-balanced meals and snacks, served daily

• Vital sign monitoring and tracking including weight, blood pressure and pulse checks

• Daily and overnight wellness checks

• Bed making and daily trash removal

• Weekly laundry, linen and housekeeping services

• Health evaluation upon move-in, change of condition or as needed

• Daily social engagement, educational opportunities and outings

• Scheduled transportation to medical appointments

• All utilities except phone, including expanded basic cable television

Level of Care

During your respite stay at Someren Glen, your daily rate includes all daily care assistance as needed during the respite period. One of our licensed nurses will conduct an assessment to determine what your individual needs are. Your rate is a fixed rate and will not fluctuate with needs. Care will include:

• Daily dressing and grooming assistance

• Reminders and escorts to and from meals and community activities

• Assistance with showers and bathing

• Support with bathroom and toileting routines

• Medication ordering, administration and tracking by qualified professionals, through community­

preferred pharmacy
